If you wish to order by mail and pay by check, send a letter containing the information from the form below, with the check payment drawn against a US bank in US dollars, to the address listed in the Contact Information button.
If you wish to order by fax and pay by credit card, send a letter (or printed copy of the form below) containing the information from the form below to the fax number listed in the Contact Information button.

Each proceedings printed copy is available for $50 plus shipping.

Shipping of printed copies is $5 each "book rate" within the US. Air shipping is $11 each in North America or $19 each elsewhere in the world.
